The Patient Experience
Patients are greeted by a real person on the First Tracks care management team. We connect however is most comfortable for the patient – text, email, video or audio call.
The patient completes a proprietary intake based on validated measures. Using the First Tracks Care Pathways, optimal treatment options are identified.
Treatment options are explained to the patient and they receive a warm handoff to their treating clinician. We check in during every step of the process and adjust their care pathway as needed.
